Case Managers work with individuals and families to understand their illness, injury and health issues to determine care needs and develop a care plan for the best possible outcome.

Case Management Process
The Case Management process consists of several phases to ensure the client’s needs and interests are met.
The phases of the process are:
Assessing: During a comprehensive In-Home Assessment, the unique care needs and issues of the client and family are determined.
Stratifying Risk: Any potential risks and issues are identified to predict future care needs to prioritize the management of care in preventing any long-term problems and ensuring the best outcome.
Care Plan: A home care plan with recommendations for other potential services is developed. The Case Manager will provide any needed referrals for speciality services such as dietary, medical, legal, or financial.
Implementing and Care Coordination: The Case Manager with coordinated all our in-home care services with the client, family, and caregivers. As well as provide support for family members, the client, and others involved in the caregiving process.
Following-Up and Communication: As the care point person, the Case Manager will keep in constant communication with the client, family, and caregivers to ensure that all care needs are being met. Unlike nursing homes and assisted care facilities where services and costs are relatively fixed, in-home care is highly flexible, and care services can be increased and decreased quickly to meet changing needs and situations.
Transitioning: The Case Manager will assist the family should the client need to be transitioned to a facility.
