Required Daily COVID-19 Client Questions

Until further notice, we are required to get answers to these four questions daily from our clients:

1. Has a fever (Higher than 100.03 degrees) or new respiratory symptoms such cough, shortness of breath, or sore throat?

Yes or No

2. Has traveled to a COVID-19 affected area (outside the US) in the past 14 days?

Yes or No

3. Has had contact with a person (live with or contact within 6 ft for 15 mins) diagnosed with COVID-19 in the past 14 days?

Yes or No

4. Has been diagnosed with COVID-19 or told by a healthcare provider they may have or have COVID-19?

Yes or No

Please ask the client and text the answers to the four questions to Rebecca daily at 781-366-7577.

You can text “all 4 no”

Please also add the client’s name, for example, text “Brady all 4 no”.

If you ever have Yes for an answer, please call Rebecca imminently.
